Frau Grünauer, Herr Nesic, das Klinische-Chemie Team im ILM hat sich vor einigen Monaten entschieden den BÜHLMANN fCAL® turbo Test auf dem Roche Cobas® c501 in Ihrem Labor zu adaptieren. Aus welchem Grund?
Das Stadtspital Triemli wollte den Arbeitsablauf im Labor sowie den Service gegenüber Einsendern für die Calprotectin Bestimmung optimieren. Der fCAL turbo Test der Firma BÜHLMANN bietet einen entscheidend breiteren Messbereich von 20 – 8`000 ug/g als unsere vorherige ELISA Messmethode, deren cut-off bei 600 ug/g lag. Der fCAL turbo Test eignet sich hervorragend für Einzelbestimmungen im Random Access Modus. Wir konnten die Testergebnisse unseren Gastroenterologinnen und Gastroenterologen bereits innerhalb einer Stunde nach Probeneingang liefern. Vorher mussten wir die Proben sammeln und haben die Bestimmung nur 2mal pro Woche durchgeführt. Der Kostenfaktor spielte ebenfalls eine Rolle: im Vergleich konnten wir mit dem Random Access Modus, Einsparungen erzielen.
Wie waren Ihre Erfahrungen mit der Adaption des BÜHLMANN fCAL® turbo Tests auf dem Roche Cobas® c501 klinisch-chemischen Analyzer?
Die Adaption des fCAL turbo Tests auf dem Roche Cobas® c501 Gerät war sehr einfach und verlief vollkommen problemlos. Die Adaption wurde zwischen unserer Geräteverantwortlichen im Labor und den Firmen BÜHLMANN und Roche in Teamarbeit ausgeführt. Nach nur einer Stunde Unterbrechung konnten wir unsere Routinearbeit wieder aufnehmen.
Welche Vorteile sehen Sie in der routinemässigen Anwendung des BÜHLMANN fCAL® turbo Tests?
Der BÜHLMANN fCAL® turbo Test ist sehr einfach, schnell und flexibel und jederzeit durchführbar. Prä-analytisch sehen wir bereits einen enormen Vorteil: Vorher mussten wir die Proben bis zu 5 Tage gekühlt lagern und dann extrahieren. Heute wird jede einzelne Stuhlprobe sofort nach Eingang extrahiert, anschliessend für 10 min bei 3`000 g zentrifugiert und im Gerät gemessen. Nach nur 10 min haben wir das Testergebnis. Die Kalibration ist über 4-6 Wochen stabil. Die niedrigen und hohen Kontrollen, welche wir täglich im Rahmen unserer Qualitätskontrolle durchführen, liegen seitdem wir den Test eingeführt haben immer im Kontrollbereich.
Welche Extraktionsmethode verwenden Sie? Wie ist die Handhabung dieser Methode?
Wir verwenden die BÜHLMANN CALEX® Cap Extraktionsmethode. Die bereits mit Puffer befüllten Röhrchen sind gebrauchsfertig und die Extraktion kann in drei einfachen Schritten durchgeführt werden: 1. Entnahme der Stuhlprobe mit der Dosierspitze, 2. Verschliessen des Röhrchens und anschliessendes vortexen bis die Probe vollständig gelöst ist und 3. Zentrifugation für 10 Minuten – und das CALEX® Röhrchen ist fertig fürs Gerät. Die Extraktion mit CALEX® Cap ist somit schnell, einfach und sauber.
Haben Sie Probleme oder Interferenzen mit anderen klinisch-chemischen Tests beobachtet, nachdem Sie Stuhlextrakte auf dem Cobas® c501 Analyzer gebracht haben? Wie sind hier Ihre Erfahrungen?
Am Anfang hatten wir Bedenken, Stuhlextrakte auf unserem klinisch-chemischen Analysegerät zu messen. Daher haben wir die Testergebnisse aller klinisch-chemischen Parameter auf dem Roche Cobas® c501 nach der Adaption des fCAL turbo Tests kritisch und sehr genau beobachtet. Wir konnten absolut keine Interferenzen in keinem der Assays feststellen. Da die zentrifugierten Stuhlextrakte frei von Feststoffen sind, haben wir auch nie ein Clotting-Problem beobachtet.
Warum würden Sie anderen Anwender die Adaption des BÜHLMANN fCAL® turbo Tests auf Ihrem klinisch-chemischen Analyzer empfehlen?
Der BÜHLMANN fCAL® turbo Test hat uns die Arbeit im Labor enorm erleichtert und den Service gegenüber unseren Einsendern extrem verbessert: die Testergebnisse sind zuverlässig und entsprechen dem klinischen Bild. Die flexible und schnelle Bestimmung im Random Access Modus ist eine ideale Lösung für unser Labor und garantiert heute Testergebnisse innerhalb von 1 Stunde nach Probeneingang. Die Stuhlextraktion mit dem CALEX® Cap ist sauber, schnell und effizient. Die Testreagenzien und Kalibratoren sind sehr stabil, täglich gemessene Kontrollen sind immer im Messbereich. Mit dem erweiterten Messbereich von 20 – 8`000 ug/g können unsere Einsender nun eine gezieltere und genauere Verlaufskontrolle durchführen.
Wie zufrieden sind Sie mit dem Service und dem Support von BÜHLMANN?
Der Service und der Support der Firma BÜHLMANN bei der Beratung und während der Adaption des fCAL turbo Tests auf unser Gerät im Labor funktionierte reibungslos und war sehr gut. Da wir seit einem Jahr ohne jegliche Probleme in der klinischen Routine arbeiten, hatten wir seitdem kein Bedarf für einen weiteren Support von BÜHLMANN. Wir sind sehr zufrieden.

Madame Grünauer, Monsieur Nesic, votre équipe du laboratoire de chimie clinique de l’hôpital de Triemli a décidé d’utiliser le test BÜHLMANN fCAL® turbo sur le Cobas® c501 de la société Roche. Pourriez-vous nous indiquer les raisons de ce choix ?
L’hôpital Triemli a voulu optimiser l’analyse de la calprotectine en améliorant le service offert aux clients et le travail de routine dans le laboratoire. Le test fCAL® turbo de BÜHLMANN offre une gamme de mesure allant de 20 – 8`000 µg/g ce qui représente un avantage considérable par rapport à la méthode ELISA, que notre laboratoire utilisait avant, atteignant une valeur maximale de 600 µg/g. Le test fCAL turbo est parfait pour des déterminations unitaires en accès aléatoire. Nous sommes désormais capables de rendre nos résultats aux gastro-entérologues en une heure seulement après réception des échantillons. Auparavant, nous devions regrouper les échantillons et n’effectuions les mesures de calprotectine que deux fois par semaine. Un autre facteur ayant joué un grand rôle dans notre décision est le coût : grâce à ce mode d’accès aléatoire, nous faisons des économies.
Quelles sont vos expériences avec l’application du test BÜHLMANN fCAL® turbo sur l’instrument de chimie clinique Roche Cobas® c501 ?
L’application du test fCAL turbo sur le Roche Cobas® c501 a été très simple et s’est passée sans problème. Cette dernière a été mise en place grâce à un travail d’équipe entre les sociétés BÜHLMANN et Roche, et avec le soutien également de leurs spécialistes d’applications. Il n’a suffi que d’une heure d’interruption pour établir cette méthode et nous avons pu reprendre notre travail de routine sur l’instrument.
Quels sont les avantage pour vous d’utiliser le test BÜHLMANN fCAL® turbo en routine ?
Le test fCAL® turbo de BÜHLMANN est très simple, rapide, flexible et peut être réalisé à tout moment. Du point de vue pré-analytique, cela présente déjà un grand avantage car avant, il fallait conserver les échantillons pendant 5 jours au réfrigérateur et ensuite effectuer l’extraction. Grâce à ce nouveau test nous pouvons maintenant extraire chaque échantillon de selles dès sa réception, le centrifuger pendant 10 min à 3’000 g et directement l’analyser. En 10 minutes, nous avons le résultat du test. Les calibrateurs sont stables pendant 4 à 6 semaines. Les contrôles de qualité (concentrations basse et haute) que nous effectuons chaque jour ont toujours été dans la gamme de spécifications attendues.
Quelle méthode d’extraction utilisez-vous ? Que pensez-vous de la manipulation de cette méthode ?
Nous utilisons le dispositif CALEX® Cap de BÜHLMANN pour effectuer l’extraction des selles. Les tubes sont remplis de tampon, prêts à l’utilisation et cette dernière se fait en trois étapes : 1. introduire la pointe rainurée dans l’échantillon de selles, 2. fermer le tube et vortexter pour homogénéiser l’échantillon et 3. centrifuger pendant 10 minutes. Le tube CALEX® est prêt à être mis à bord de l’instrument. L’extraction avec le CALEX® Cap est donc rapide, simple et propre.
Avez-vous observé ou rencontré des problèmes ou des interférences avec d’autres tests de chimie clinique depuis que vous avez mis le nouveau test fCAL® turbo de BÜHLMANN sur votre automate Cobas® c501 ? Quelles sont vos expériences ?
Au début, nous avions des hésitations à mettre à bord de notre automate de chimie clinique des extraits de selle. Nous avons donc revu en détail les résultats de tous les paramètres de chimie clinique sur le Roche Cobas® c501 après l’application du fCAL turbo et n’avons observé aucun problème ni interférence avec d’autres tests. Après l’étape de centrifugation toutes les particules sont éliminées et nous n’avons jamais observé un problème d’obstruction.
Pourquoi recommanderiez-vous à d’autres utilisateurs d’adaptation des tests BÜHLMANN fCAL® turbo sur votre automate de chimie clinique ?
Le test BÜHLMANN fCAL® turbo a énormément facilité le travail du laboratoire et il permet d’améliorer le service que nous proposons à nos médecins. Les résultats des tests sont fiables et conformes à la clinique. La détermination de la calprotectine en mode accès aléatoire est flexible et rapide et idéale pour notre laboratoire car nous pouvons maintenant rendre les résultats des tests en seulement 1 heure après réception de l’échantillon. L’extraction de selles avec le CALEX® Cap est propre, rapide et efficace. Les réactifs et calibrateurs sont très stables, les contrôles de qualité quotidiens sont toujours dans la gamme de mesure. Grâce à la gamme étendue de 20 – 8`000 µg/g, nos médecins peuvent maintenant également effectuer un suivi plus ciblé et plus précis de leurs patients.
Etes-vous satisfaits du service et du soutien de la société BÜHLMANN ?
Le service et le soutien de la société BÜHLMANN lors de la consultation et de l’adaptation des tests fCAL turbo dans notre laboratoire a très bien fonctionné et s’est déroulée sans difficulté. Après une année de pratique clinique en routine avec le fCAL turbo, nous travaillons sans aucun problème et nous n’avons pas eu besoin de soutien supplémentaire de la part de BÜHLMANN. Nous sommes vraiment très satisfaits.

Signora Grünauer, Signor Nesic, il team di chimica clinica dell’ IML pochi mesi fa ha deciso di adattare il test BÜHLMANN fCAL® turbo sul Roche Cobas® c501 nel vostro laboratorio. Per quale motivo?
L’ospedale Triemli ha voluto ottimizzare il flusso di lavoro in laboratorio e il servizio per i richiedenti del test della determinazione della calprotectina. Il test fCAL turbo dell’azienda BÜHLMANN, con un range di analisi di 20-8`000 ug/g, offre una gamma di determinazione fondamentalmente più ampia in confronto al nostro precedente metodo di misurazione in ELISA, il cui limite massimo di determinazione era di 600 ug/g. Il test fCAL turbo è ideale per singole determinazioni in modalità operativa Random Access. Possiamo così fornire i risultati del test per i nostri gastroenterologi in appena un’ora dal ricevimento del campione. In precedenza dovevamo raccogliere i campioni, conservarli e effettuare la determinazione solo 2 volte a settimana. Anche il fattore di costo ha svolto un ruolo: in confronto abbiamo raggiunto un risparmio con la modalità operativa Random Access.
Qual è stata la sua esperienza con l’adattamento del test BÜHLMANN fCAL® turbo su analizzatore di chimica clinica Roche Cobas® c501?
L’adattamento del test fCAL turbo sul dispositivo Roche Cobas® c501 è stato molto semplice ed è stato realizzato senza problemi. L’adattamento è stato effettuato con un lavoro di squadra tra i nostri responsabili nel laboratorio e le società BÜHLMANN e Roche. Dopo solo un’ora di interruzione dello strumento, siamo stati in grado di riprendere il nostro lavoro di routine.
Quali vantaggi vede nell’applicazione in routine del test BÜHLMANN fCAL® turbo?
Il test BÜHLMANN fCAL® turbo è molto semplice, veloce e flessibile e praticabile in qualsiasi momento. A livello di preanalitica stiamo già vedendo un enorme vantaggio: in precedenza dovevamo conservare I campioni in frigo fino a 5 giorni e poi procedere con l’estrazione. Oggi ogni campione di feci viene estratto immediatamente dopo la ricezione, centrifugato per 10 min a 3`000 g e misurato nel dispositivo. Dopo soli 10 minuti abbiamo il risultato del test. La calibrazione è stabile per 4-6 settimane. I controlli bassi e alti che eseguiamo ogni giorno come parte del nostro controllo di qualità sono sempre nel range di controllo, da quando abbiamo introdotto il test.
Quale metodo di estrazione utilizzate? Com’ è la maneggevolezza di questo metodo?
Usiamo il metodo di estrazione BÜHLMANN CALEX® Cap. I dispositivi già riempiti con tampone di estrazione sono pronti per l’uso e l’estrazione può essere effettuata in tre semplici passaggi: 1. Prelevamento del campione di feci con il tappo di campionamento, 2. Chiusura del dispositivo e successiva agitazione su un agitatore Vortex, finché le scanalature non risultano completamente prive di feci e 3. Centrifugazione per 10 minuti – ed il dispositivo CALEX® è pronto per l’analisi. L’ estrazione con il CALEX® Cap è quindi veloce, semplice e pulita.
Avete notato problemi o interferenze con altri test di chimica clinica dopo aver portato estratti fecali sul Cobas® C501 Analyzer? Qual è la vostra esperienza?
Inizialmente eravamo preoccupati del dover misurare estratti di feci sul nostro analizzatore di chimica clinica. Per questo, abbiamo controllato con estrema attenzione i risultati di tutti i parametri di chimica clinica dopo l’adattamento del test fCAL turbo sul Roche Cobas® C501. Non abbiamo notato assolutamente alcuna interferenza in una qualsiasi delle analisi. Dato che gli estratti fecali centrifugati sono privi di solidi, non abbiamo neanche mai osservato un problema di coagulazione.
Perché raccomandereste ad altri utenti l’adattamento del test BÜHLMANN fCAL® turbo sul loro analizzatore di chimica clinica?
Il test BÜHLMANN fCAL® turbo ha reso notevolmente più facile il nostro lavoro in laboratorio e migliorato il servizio verso i nostri richiedenti: i risultati del test sono affidabili e conformi con il quadro clinico. La determinazione flessibile e rapida nella modalità operativa Random Access è una soluzione ideale per il nostro laboratorio e garantisce oggi i risultati del test entro un’ora dal ricevimento del campione. L’estrazione delle feci con il CALEX® Cap è pulita, veloce ed efficiente. I reagenti e i calibratori sono molto stabili, i controlli misurati ogni giorno sono sempre nel campo di misura. Con il range esteso di 20 – 8`000 ug / g i nostri richiedenti ora possono effettuare un follow-up più mirato e più accurato.
Quanto siete soddisfatti con il servizio e il supporto di BÜHLMANN?
Il servizio e supporto dell’azienda BÜHLMANN durante la consultazione e durante l’adattamento del test fCAL turbo sulle nostre attrezzature in laboratorio ha funzionato senza intoppi ed è stato molto buono. Dato che da un anno lavoriamo senza problemi nella routine clinica, da quel momento non abbiamo avuto bisogno di un ulteriore supporto da BÜHLMANN. Siamo molto soddisfatti.

Mrs. Grünauer and Mr. Nesic, several months ago you decided to implement the BÜHLMANN fCAL® turbo on your Roche cobas® c501 system. Can you tell me why?
The City Hospital Triemli wanted to optimize the workflow in the laboratory as well as the referral service for Calprotectin testing. The BÜHLMANN fCAL® turbo assay offers an essential expanded measuring range from 20 to 8`000 µg/g compared to our former ELISA method that had a cut-off at 600 µg/g. The fCAL turbo assay is suited for single determinations in random access mode. Now we can provide our Gastroenterologists with Calprotectin results 1h after sample receipt. Before we had to batch the samples and performed the Calprotectin ELISA twice per week. The expense factor also plays a major role: compared to the ELISA method we were able to reduce the costs by using the random access mode.
Tell me about your experience implementing the BÜHLMANN fCAL® turbo assay on your Roche Cobas® c501 clinical chemistry analyzer?
The implementation of the fCAL turbo assay on the Roche Cobas® c501 device was very easy and worked smoothly. The adaption was performed in conjunction with our device specialist as well as both companies BÜHLMANN and Roche. After only 1h of break, we could restart our routine work!
What advantages do you see in the routine use of the BÜHLMANN fCAL® turbo?
The BÜHLMANN fCAL® turbo assay is very easy, fast, flexible, and feasible at any time. We also see big advantages in the pre-analytics: previously, we had to store the stool samples for 5 days before extraction. Now we extract each sample right after receipt and after centrifugation for 10 min at 3`000 g we can easily load the extract on the device; 10 minutes later we have the result. The calibration is stable for 4-6 weeks. According to our quality assurance, we measure the low and the high control daily and both controls are always within range.
Which extraction method do you use? What is your impression of the technique?
We are using the BÜHLMANN CALEX® Cap extraction device. The pre-filled tubes are ready-to-use and the extraction is performed in three easy steps: 1. Sampling of the stool with the pin, 2. Placing of the sampling pin back into funnel, pushing it back until it „clicks“ and dissolving the sample using a Vortex, and 3. Centrifugation for 10 min – the CALEX tube is now ready for the instrument. The extraction with the CALEX® Cap is fast, easy, and clean.
Have you ever experienced any interferences with other clinical chemistry tests running on your Roche Cobas® c501 originating from using stool extracts and the BÜHLMANN fCAL® turbo?
At first, we were nervous about measuring stool extracts on our clinical chemistry analyzer. Therefore we critically reviewed all our other clinical chemistry parameters on the Roche Cobas® c501 after we had introduced the fCAL turbo assay. No interferences or clotting problems were observed because the centrifuged extracts are used that are free from undigested fibres.
Would you recommend the use of the BÜHLMANN fCAL® turbo on clinical chemistry analyzers to other customers and why?
Certainly; the BÜHLMANN fCAL® turbo assay eased our routine work in the laboratory tremendously and improved our service for senders: the results are reliable and match the clinical data. The flexible and fast determination in random access mode is the perfect solution for our laboratory that guarantees a result within 1h after sample receipt. The extraction using CALEX® Cap is clean, fast, and efficient. Reagents and calibrators are stable; both daily measured controls are always within range. Monitoring of IBD is more accurate now because of the extended measuring range from 20 µg/g to 8`000 µg/g.
Are you satisfied with the service and support by BÜHLMANN?
The service and support were excellent during consultation period and the implementation of fCAL turbo assay on our laboratory instrument worked smoothly. The assay has been in routine use for a year now and we have absolutely no problems; that´s why we have had no need for further support by BÜHLMANN. We are very satisfied.
